Why Balance Declines as We Age — and How Physiotherapy Can Help

Balance isn’t just about standing on one leg — it’s your body’s ability to control your centre of mass over your base of support. Good balance helps you walk, climb stairs, reach overhead, bend, and respond safely to unexpected slips. It’s foundational to independence and confidence in daily life.
Many people notice that balance becomes more challenging as they get older. That can be worrying — and it’s very common. The good news is that age-related balance changes are not inevitable declines you simply have to accept. Why Balance Changes with Age
Balance is a complex skill that depends on multiple body systems working together:
1. Sensory Systems Become Less Reliable
Your brain uses three key sources of feedback to understand body position:
Vision — helps orient you in space
Vestibular system (inner ear) — senses head movement and gravity
Somatosensory feedback (touch/joint position sense) — tells you where your body is without looking
With age:
Vision often becomes less sharp and slower to process information
Inner ear function declines (vestibular hair cells decrease)
Sensation in the feet and joints can diminish
High-quality research shows that reduced sensitivity in the feet and joints is associated with poorer balance and increased falls risk in older adults. That doesn’t mean it’s untreatable — just that the body needs more reliable input from other systems and movement training to compensate.
2. Muscle Strength and Power Decline
From mid-life onwards, muscle mass and strength tend to decrease — a process called sarcopenia. This affects especially the legs and core, which are essential for correcting imbalance when you slip or trip.
Notably, it’s not just how strong you are, but how quickly you can produce force (power) that protects you from falls. Research supports that exercise programs focused on strength and power reduce falls in older adults.
3. Reaction Time Slows
As we age, nerve conduction and processing speed can slow slightly. This can make reactions to sudden perturbations (like stumbling on uneven ground) slower, increasing the likelihood of imbalance becoming a fall.
4. Pain, Fear of Falling, and Reduced Movement
Pain from joints (for example hips, knees, or back) is common with age and can lead people to move less. Reduced movement in turn:
Weakens muscles
Reduces confidence in movement
Causes stiff joints
Narrows the range of sensory experiences balance systems rely on
Psychological factors like fear of falling can also limit activity and inadvertently weaken balance responses.
Is Balance Decline Inevitable?
Some decline in certain systems with age is expected — but how well you function is shaped by what you do. We now have strong evidence that balance and mobility can be improved at almost any age with the right approach.
