Running Pain-Free After a Hamstring Injury? When Are You Ready to Run Faster?
Being able to jog without pain after a hamstring injury is an encouraging milestone.
But does pain-free running mean your hamstring has completely recovered?
Not necessarily.
One of the challenges with hamstring rehabilitation is that comfortable jogging and high-speed running place very different demands on the hamstring muscles. You may therefore reach a stage where normal walking, strengthening exercises and slower running feel good, while your hamstring has not yet demonstrated the capacity required for faster running, sprinting or sport.
Rather than relying on pain alone, physiotherapists will usually consider a combination of symptoms, strength, response to loading and running capacity when deciding how rehabilitation should progress.
Can I run if my hamstring doesn't hurt?
Potentially, depending on the nature and stage of your injury.
Pain-free jogging is generally a positive sign during hamstring rehabilitation. However, the absence of pain during an easy run shouldn't be treated as the only measure of recovery.
Current international consensus on hamstring rehabilitation supports a criteria-based approach to returning to running, rather than relying purely on the amount of time since the injury occurred.
Assessment may include:
your current pain and symptoms
tenderness around the injured area
hamstring strength
how the hamstring responds to different types of muscle contraction
tolerance to progressively greater loads
running capacity
the speed and sporting demands you ultimately need to return to.
The appropriate criteria will vary between individuals and injuries.
Why can jogging feel fine before sprinting?
Running isn't one consistent physical demand.
The forces, muscle activity and rate at which the hamstrings have to produce and absorb force change as running speed increases.
During high-speed running, the hamstrings have an important role in controlling the leg during the late swing phase of the running cycle, before the foot contacts the ground. They also contribute during the transition into stance.
As running speed increases, the mechanical demands placed on the hamstrings increase considerably.
This helps explain why someone may tolerate a comfortable jog without symptoms but not yet be ready for repeated accelerations or sprinting.
Pain-free jogging tells us something useful. It doesn't tell us everything.
What does a physiotherapist assess after a hamstring injury?
There isn't one test that determines whether a hamstring has recovered.
A physiotherapy assessment generally combines several pieces of information.
1. Pain and tenderness
We want to understand whether symptoms are still present and how the hamstring responds to examination and loading.
This can include tenderness when the injured area is palpated and symptoms produced during particular movements or muscle contractions.
Symptoms are considered alongside the rest of the assessment rather than being interpreted in isolation.
2. Hamstring strength
Strength is an important part of hamstring rehabilitation.
Depending on the injury and stage of rehabilitation, strength may be assessed with the knee and hip in different positions and through different types of contraction.
Comparing the injured and uninjured legs can provide useful information, although perfect symmetry isn't a universal requirement and the opposite leg shouldn't automatically be assumed to represent someone's normal pre-injury capacity.
Later-stage rehabilitation will commonly include progressively greater hamstring loading, including strengthening at longer muscle lengths and eccentric loading where appropriate.
3. How the hamstring tolerates contraction and loading
We are interested not only in whether you can contract the hamstring, but also in how it behaves as the demands increase.
For example, someone may comfortably perform an introductory hamstring exercise but experience symptoms or demonstrate reduced capacity with more demanding loading.
Rehabilitation therefore generally progresses from what the hamstring can currently tolerate towards the capacity required for the person's sport or activity.
4. Running progression
Running itself is an important component of rehabilitation for many hamstring injuries.
Returning to running shouldn't necessarily mean going directly from jogging to maximum-speed sprinting.
A progression may involve gradually increasing:
running speed
distance
acceleration
exposure to higher-speed running
sport-specific movements
overall training volume.
The exact progression will depend on the individual and the activity they are returning to.
A recreational distance runner, footballer and competitive sprinter, for example, have very different end-stage requirements.
How do you know when you're ready to sprint after a hamstring injury?
There is currently no single universally accepted test or percentage that can guarantee someone is ready to sprint.
That's important because rehabilitation advice can sometimes make return-to-sport testing sound more precise than the evidence allows.
Instead, the decision is generally made by combining information.
Before progressing towards high-speed running, we may consider whether you have:
tolerated the preceding stages of running
minimal or no relevant symptoms with appropriate testing
adequate hamstring strength and loading capacity
tolerated progressively demanding hamstring exercises
regained the running capacity required for the next stage
responded appropriately following previous training sessions.
International hamstring consensus recommendations support completing submaximal running before progressing to sprinting. They also support pain-free sprinting as an important consideration when progressing towards return to sport.
The progression should ultimately reflect the demands you need to return to.
Is strength compared with the other leg important?
Yes, but it needs context.
Comparing hamstring strength between legs can help identify a substantial deficit following injury.
However, a simple percentage comparison isn't a perfect measure of recovery.
The opposite leg may also have changed during a period of reduced training, and athletes can have differences between sides even when uninjured. Current expert consensus has therefore not established one universal limb-symmetry percentage that determines whether every athlete is ready to return.
We are more interested in building a broader picture of your capacity than simply trying to achieve one number.
Does being pain-free mean the hamstring has healed?
This is where terminology matters.
Pain is useful clinical information, but absence of pain isn't the same thing as demonstrating readiness for every activity.
If you can jog pain-free, we know your hamstring has tolerated that particular load at that particular speed.
It doesn't automatically tell us how it will respond to a maximal sprint, sudden acceleration, kicking a football or repeated high-speed efforts.
For this reason, rather than simply asking:
“Does it hurt?”
we also want to ask:
“What can the hamstring currently tolerate, and what does it still need to tolerate before you return to your activity?”
That is a more useful way to think about hamstring rehabilitation.
Why is high-speed running important in hamstring rehabilitation?
If your goal involves sprinting or a running-based sport, eventually the rehabilitation program needs to prepare you for those demands.
Gym-based strengthening is valuable, but it doesn't completely reproduce the demands of high-speed running.
Running and sprinting are therefore considered important components of later-stage hamstring rehabilitation for athletes who need these capacities.
The key word is progression.
The aim isn't to avoid sprinting indefinitely after a hamstring injury. It is to progressively build sufficient capacity before exposing the hamstring to the highest running demands.
What exercises are used for hamstring rehabilitation?
There isn't one “best hamstring exercise” for every person or every stage of recovery.
Exercise selection may include combinations of:
knee-flexion strengthening
hip-extension exercises
eccentric hamstring strengthening
exercises performed at progressively longer muscle lengths
trunk, hip and lower-limb strengthening
running drills
progressive running exposure
sport-specific rehabilitation.
Recent research continues to support appropriately progressed strengthening, including eccentric and lengthening exercise, as an important component of hamstring rehabilitation.
However, exercise selection, dosage and timing should reflect the individual injury rather than simply applying the same protocol to everyone.
How quickly should running speed increase?
There isn't a universal timetable.
This is another area where a generic “week 1, week 2, week 3” approach can be misleading.
Progression depends on factors including:
the nature and severity of the injury
symptoms
strength
previous running exposure
response to rehabilitation
sport
position or event
training history
the speed and volume ultimately required.
A useful principle is to progressively expose the hamstring to greater running demands while monitoring how it responds.
Someone returning to recreational jogging may have a very different rehabilitation endpoint from someone who needs repeated maximum-speed sprinting for competitive sport.
Frequently asked questions
If I can jog without pain, can I start sprinting?
Not automatically. Pain-free jogging is a useful milestone, but sprinting places substantially greater demands on the hamstrings. Strength, symptoms, loading capacity and tolerance of progressively faster running should also be considered.
Should hamstring rehabilitation be completely pain-free?
Not necessarily at every stage. Some rehabilitation exercise may be performed within an acceptable symptom threshold depending on the individual and stage of rehabilitation. However, current international expert consensus favours pain-free sprinting when progressing towards return to sport.
Should both hamstrings have exactly the same strength before I run?
Not necessarily. Side-to-side comparison can be useful, but there isn't one universally accepted strength-symmetry percentage that determines readiness for every person. Strength needs to be considered alongside symptoms, running capacity and the demands of the activity.
Is jogging enough to rehabilitate a hamstring injury?
If your goal is returning to a sport involving high-speed running, usually not. Jogging doesn't reproduce the demands of sprinting. Progressive strengthening and appropriately graded exposure to faster running are commonly important parts of later-stage rehabilitation.
Can a physiotherapist assess whether I'm ready to return to running?
A physiotherapist can assess your symptoms, strength and functional capacity and use this information alongside your response to progressive running to help guide your rehabilitation. Return-to-sport decisions are individual and should consider the demands of the activity you are returning to.
