Heat or Ice for an Injury? Which One Should You Use?

“Should I use heat or ice?” is one of the most common questions we hear as physiotherapists.
For years, the simple advice was often ice for a new injury and heat for an old injury.
The reality is a little more nuanced.
Ice can be useful following some acute injuries, particularly for short-term pain relief. Heat can be useful for stiffness and some ongoing musculoskeletal problems. However, there are also situations where heat should be avoided — particularly immediately following an acute injury where bleeding and swelling may still be occurring.
So how do you decide?
Should You Use Heat on a New Injury?
If you've just sustained an acute traumatic injury — such as a muscle tear, significant ankle sprain or direct blow resulting in bruising — heat is generally not our first choice during the early stages.
Heat increases local blood flow.
If an injury has caused damage to small blood vessels and bleeding into the tissues, increasing blood flow during this early period may potentially contribute to further bleeding and swelling.
This is why heat is commonly avoided during approximately the first 48–72 hours following an acute soft-tissue injury, particularly when there is obvious swelling, bruising, heat or inflammation.
This doesn't mean every painful condition requires you to wait exactly 72 hours before using heat. Different injuries behave differently.
The important distinction is whether you're dealing with a fresh traumatic injury where bleeding and swelling may still be developing versus an ongoing musculoskeletal problem where this is no longer a concern.
Should You Use Ice After an Acute Injury?
Ice may be useful following an acute injury, but our reasoning for using it has changed.
Traditionally, people were often told to ice an injury aggressively to reduce inflammation.
We now understand that inflammation is also part of the body's normal healing response. Completely suppressing that process isn't necessarily the goal.
The clearest reason to use ice following an acute musculoskeletal injury is generally short-term pain relief.
Cooling the area can temporarily decrease pain and may make the injury more comfortable during the early stages.
There is also some evidence suggesting that very early use of ice may help limit the development of a haematoma following certain injuries. However, there isn't strong evidence that repeatedly icing an injury for days will make the tissue heal faster.
Therefore, we don't necessarily need to continually ice every injury simply because it is swollen.
Can You Ice an Injury Too Much?
Potentially.
Emerging research has questioned whether excessive or prolonged icing could interfere with some of the biological processes involved in tissue repair.
Much of this evidence comes from laboratory and animal research, so we need to be careful about directly applying it to humans.
However, it does reinforce an important point:
More ice isn't necessarily better.
If ice helps control pain during the early stages of an injury, it can be useful.
But continually icing an injury for long periods isn't necessarily going to accelerate your recovery.
When Is Heat More Appropriate?
Heat can become more useful when active bleeding and acute swelling are no longer concerns.
For example, some people find heat helpful for:
muscle stiffness
persistent muscle soreness
ongoing joint stiffness
chronic musculoskeletal pain
feeling stiff before movement or exercise.
Heat may temporarily increase local circulation and can make an area feel more comfortable and easier to move.
This can be particularly helpful when stiffness rather than acute swelling is the main problem.
Again, heat should be considered a symptom-management tool, rather than something that necessarily fixes the underlying cause of the problem.
What About Heat or Ice After 48–72 Hours?
This is where the decision becomes more individual.
Once you're beyond the early phase of an injury and there is no concern about ongoing bleeding or rapidly increasing swelling, either heat or ice may sometimes be used for symptom relief.
For example:
If an area remains painful and irritated after activity, you may prefer ice.
If an area feels predominantly stiff and improves once warmed up, heat may feel more beneficial.
At this stage, personal response becomes more relevant.
However, this is different from saying that heat and ice are interchangeable immediately after an acute injury.
Heat or Ice: What Is Actually More Important?
For most musculoskeletal injuries, this is the bigger question.
Heat and ice may help your symptoms, but neither one rebuilds the capacity of an injured muscle, tendon or joint.
As the early injury settles, rehabilitation increasingly becomes the priority.
Depending on the injury, this may involve restoring:
Movement
After an injury, joints and muscles can quickly become stiff. Appropriate movement can help restore normal function.
Strength
An injured area may lose strength and its ability to tolerate load. Progressive strengthening helps rebuild this capacity.
Load tolerance
Eventually, the injured tissue needs to tolerate the demands you're going to place on it — whether that's walking, running, working, lifting or playing sport.
Confidence
Pain can understandably make you cautious about movement. A structured rehabilitation program can progressively expose the injured area to load and help rebuild confidence.
A Simple Way to Think About Heat vs Ice
If you've sustained a fresh traumatic injury with swelling or bruising, avoid immediately applying heat because increasing blood flow may potentially increase bleeding and swelling.
Ice can be considered for short-term pain relief during the early stages, but it doesn't need to become the focus of your rehabilitation.
Once the acute stage has passed and bleeding and significant swelling are no longer concerns, heat or ice may be used depending on your symptoms.
And as your injury progresses, the question increasingly changes from:
“Should I use heat or ice?”
to:
“What does this area need to regain its normal capacity?”
That's where appropriate movement, strength and progressive loading become particularly important.
When Should You Have an Injury Assessed?
Consider having an injury assessed if you have significant swelling or bruising, cannot use the injured area normally, have substantial weakness or loss of movement, can't comfortably weight-bear, or your symptoms aren't improving as expected.
These findings can sometimes indicate a more significant muscle, tendon, ligament or bone injury.
A physiotherapy assessment can help determine what's been injured and, importantly, develop a plan for safely restoring movement, strength and function.
