Our Services Include:
Initial Assessment
Can be performed at the Rehabilitation Gym or in your home and will include the following:
- Baseline outcome measures are taken
- A tailored home exercise program is provided
- Education component including:
- The National Heart Foundation Recommendations
- Advice on exercising safely
- Guidance on how to progress your exercise program
- Information on how to monitor your exercise program’s intensity
- An update of your progress is provided to your referring doctor
- As a result of the consult you have another point of contact for further questions and professional guidance
Review
Can be performed at the Rehabilitation Gym or in your home and will include the following:
- Re-assessment of outcome measures is conducted
- Results are discussed
- Results are provided to your referring Doctor and cardiac rehabilitation nurse
- Progression of your home exercise program is provided if appropriate
- Reviews are periodically conducted at 3, 6 and 12 months during the first year to monitor progress
- Long term participants move to 6 monthly reviews
Cardiac Rehabilitation Class
Conducted in the Rehabilitation Gym and will include the following:
- Participation in a small group class allowing for the appropriate level of supervision and the ability for clients to ask questions as required
- Continued education is performed during sessions
- An individual program is designed for each client
- Each client is provided with a heart rate monitor during the class allowing them to be constantly monitored
- Heart rate monitoring throughout sessions ensures you are working at the desired level maximising obtained results
- Class environment creates team morale and increased drive to exercise and reach individual goals
